Our Assisted Living Programs (HCS/ICF)

We proudly provide two trusted assisted living programs in Texas—Home and Community-based Services (HCS) and the Intermediate Care Facility for Individuals with Intellectual or Developmental Disabilities (ICF/IID). These programs are designed to enhance the lives of individuals with intellectual and developmental disabilities (IDD) by offering personalized care and support.

Our dedicated team of professionals and caregivers works closely with families to ensure a seamless and stress-free transition, helping your loved one find a home that meets their needs. Both programs are built on a foundation of encouragement, empowerment, respect, and compassionate care, aimed at fostering growth and independence for individuals with IDD.

HCS Program (Home and Community-Based Services)

The HCS program offers a wide range of services, including:

• Housing options in 3- and 4-person group homes.
• Day programs that include transportation and employment support.
• Medical services, encompassing nursing, dental care, and physician consultations.
• Professional therapies tailored to individual needs.
• Adaptive aids and home modifications to enhance accessibility.
• Additional specialized services available under the HCS waiver.

The HCS program operates through a state-managed waiting list (referred to as the “interest list”) and is widely regarded as one of the most comprehensive support systems for individuals with intellectual disabilities in Texas.

ICF/IID Program (Intermediate Care Facility)

The ICF/IID program provides a structured support system, featuring:

• Six-person group homes for comfortable and collaborative living.
• Day services, including transportation and employment support.
• Medical care, such as nursing, dental, and physician services.
• Professional therapies to support growth and well-being.

Individuals enrolled in the ICF program may access adaptive equipment directly through their Medicaid Managed Care Plan. Most importantly, ICF services are available immediately to those with active Medicaid eligibility and an official diagnosis of an intellectual or developmental disability (or related conditions like Autism Spectrum Disorder or traumatic brain injury).

Transition Between Programs

Choosing ICF services does not affect your loved one’s placement on the HCS interest list. This allows individuals to access ICF services immediately and transition to HCS later when a spot becomes available. Many of the people currently benefitting from the HCS program began their journeys with ICF services.
